Take Control of Your Medical Care

We encourage our patients to consider creating Advance Medical Directives, no matter what stage of life they may be in. Advance Medical Directives are legal documents that allow you to express your preferences for medical treatment and care in case you become unable to communicate your wishes in the future. These documents can help ensure that your healthcare wishes are followed and respected, and can provide peace of mind for you and your loved ones.

There are two types of Advance Medical Directives: a Living Will and a Power of Attorney for Health Care. A Living Will outlines your preferences for medical treatment in case you are unable to communicate your wishes. A Power of Attorney for Health Care designates someone you trust to make medical decisions on your behalf if you become incapacitated.

Creating Advance Medical Directives is an important step in taking control of your medical care. We recommend discussing your wishes with your loved ones, as well as with your healthcare provider.
