Access Your Records
Prairie Ridge Health’s Medical Records Department is dedicated to maintaining your medical records and keeping your health information private and secure. This department may also provide copies of your records to you at your request.
Copy Fees may apply – Please contact our copy service at (920) 623-1538 for information about copy fees.
Turnaround Time – Our quality standard is to process your request for copies in 10 to 14 days. (Some restrictions may apply).

A signed, completed authorization form by you or your legal guardian is required to process your request for medical records.

Requesting Copies of a Birth Certificate
Prairie Ridge Health does not maintain copies of Birth Certificates.
Copies may be requested from the Columbia County Register of Deeds in Portage, Wisconsin. For more information, visit the Columbia County website or call (608) 742-9677.
Copies may also be requested from the State of Wisconsin, Department of Health and Family Services, Office of Vital Records. For more information about this, visit dhs.wisconsin.gov/vitalrecords/ (the order form itself is here) or call (608)266-1373.
