Prairie Ridge Health accepts over 70 insurance plans, including, but not limited to, the insurance plans listed below. Please check with your individual insurance plan regarding coverage.

* Due to coverage restrictions imposed by Dean and Quartz, we are not allowed to see patients with Dean and Quartz insurance at our Sun Prairie clinic at this time.
We understand access to care for Dean and Quartz patients is an ongoing concern. We want to provide care for you. Unfortunately, this insurance coverage issue only adds to those challenges. In many cases, Dean and Quartz allow their members to be seen at our Columbus location, which is located 15 minutes north of Sun Prairie: 1515 Park Avenue, Columbus.
What can you do?
Please consider calling your insurance company and request they include our Sun Prairie clinic in their network. Patient requests can sometimes influence provider coverage.
